Death From Medical Mistakes and Corruption in the Health Industry

In the USA alone it has been recently estimated that some 210,000 patients died from medical mistakes in a year, making this the third leading cause of death while heart disease is the first and cancer second.

The reason why it has taken so long to realise that something is not right and why so many still don't see that anything is going on can be explained like this:
On the whole, people: healthcare workers or patients or the public at large, can regard many circumstances in medicine as isolated cause and effect relationships. In other words, people generally fail to make the connections between the different cause and effect relationships.
It is necessary to make the connections so that we can step back and see the 'big picture' of what's really going on and why certain things occur. This can be likened to the analogy of looking at separated pieces of jigsaw puzzle and not having the realisation that they are somehow all connected and therefore not carrying out the task of joining up the pieces to see the big picture.
-Such has happened in the circumstances surrounding deaths from medical mistakes. This is why the corruption still continues and how the villains are allowed to get away with it. Sometimes it's fun just to put about a ½ tsp of cinnamon in a quart sized sauce pan with about 2 cups of water and let it simmer on low for a little while to let the cinnamon aroma fill the kitchen (just don't forget to turn off the stove after about 10 minutes).

The Health Concerns Among Immigrant Children

Many believe that quality health care is a fundamental human right and an antidote to the health care disparities that afflict a nation.
Children of immigrants are the fastest growing component of the child population. While immigrants are 11 percent of the total U.S. population, children of immigrants make up 22 percent of the 23.4 million children less than 6 in the United States. Correlatively, policies affecting young children and their families will increasingly be judged by their effects on the health, well-being, and school readiness of children of immigrants.
Why do immigrant's children have such limited health care access? Much of the reason is related to insurance coverage and financial access; they also face other barriers like:
-Language barriers limit access. If patients know they will have difficulty explaining their medical needs or problems to a doctor or nurse, they are less willing to seek care. About 30% of the foreign born do not speak English or do not speak it well.
-Cultural differences also play a role. The medical care system is organized differently in the US than in immigrant's home countries and managed care can be particularly difficult to navigate.
-Immigrants may have different perceptions of the need and appropriateness of medical care. The need for preventive care may be less familiar. In many countries, seeking help for mental health problems is stigmatized.
-Finally, legal status can be a concern. Unauthorized immigrants often worry that seeking care, particularly at a public facility, may lead to exposure of tier unauthorized status and increase the risk of sanctions such as deportation. Even legal immigrants may worry that using benefits could jeopardize their legal status and perhaps make it harder to gain citizenship or permanent residency.
Poverty is one of the most important predictors of negative child outcomes. Poverty rates are generally higher among children of immigrants than among children of natives and highest for young children of immigrants. Although children of immigrants are more likely to be in two-parent families, the second parent in these families is less likely to work. Immigrants earn lower wages, leading to lower family incomes. Low educational attainment and limited English proficiency which results in low performance in school and later in the labor force. At the same time, children of immigrants score lower on measures of parent-child interaction at young age.
Parents with less formal education appear less likely to enroll their children in center-based care or preschool. They may also have fewer resources at home such as books or computers to prepare their children for school.
They are also less likely to be taken on outings-such as to the grocery store, a park, a church, or a playground-than children of natives. These gaps may be associated with relatively high poverty and low educational attainment in immigrant families.
Economic hardship rates rise, and gaps between immigrant and native families narrow for food hardship measures. Over half of young, low-income children of immigrants live in families with one or more food-related problems.
Optimal childhood health is recognized as a key determinant of well-being and productivity throughout life. Early childhood in particular is increasingly viewed as a critical period in which adverse conditions and events can significantly influence subsequent physical and mental health status. For example nutritional deficits and toxic environmental exposure in both the prenatal months and in early childhood have been linked to a range of chronic conditions in adulthood, including cardiovascular disease, hypertension, lung disease, and diabetes. Associations have also been found between psychosocial stressors in childhood-such as socio emotional and economic deprivation - and the development of mental and physical problems later in life.
Additionally, childhood health is likely to influence the social and economic integration of immigrants. Experts regard delays in cognitive development during early childhood as a key reason for educational and social equality later in life. More specifically, children with cognitive and other delays in preschool are more likely to become high school dropouts and to experience delinquency, unemployment, low earnings, and dissatisfaction with their adult lives than their otherwise similar peers. Early cognitive development is also closely linked with physical health and overall well being.
Another major childhood health condition, obesity, is linked to a number of serious health issues, including hypertension, elevated blood pressure, cancer, and diabetes. Beyond its impact on physical health and mortality, obesity is related to difficulties in social adjustment, poor mental health, and lower academic achievement - factors that in turn have wide-ranging implications for children's quality of life and productivity as adults.
Additionally, children of immigrants may be at higher risk for certain conditions. For eg; pesticide exposure is a problem among children of migrant workers. Immigrant children may be less up to date on certain immunizations, particularly DtaP, Hib, and Hep-B. Health risks appear to vary greatly depending on the origins of immigrant parents, with children of Latino immigrants often the most at risk.

Has the Health and Safety Pendulum Swung Too Far?

Mark the 1st April 2015 in your diaries now. This is the date that the revised New Zealand Health and Safety at Work Act comes into force. If the NZ government has anything to do with it (and after all, it wrote the thing; well the Australian Government did!) this Act will change the face of Health and Safety as we know it.
With some aggressive targets of reducing work-place deaths by 25% over the next five years, this bill has wide-reaching impact. In summary some of the major changes will be:
An expanding vocabulary
The Act will introduce some new concepts (and words) to the life of virtually everyone involved in enterprise. Firstly there's the concept of a 'worker'. Note that this is nothing to do with their employment status. The fact that someone in your office or factory is self-employed has no bearing. They are a worker in your workplace.
A new acronym, the "PCBU" rears its head. This a Person in Charge of a Business or Undertaking and might be a company or corporation. Note the absence of the word 'employer'. The PCBU's main responsibility it to ensure the Health and Safety of all people at the worksite, including employees, clients, visitors, contractors (and their subbies). PCBUs can be companies (and any person who is deemed to be in a position of a Director, whatever their actual title), partnerships and body corps.
The PCBU will have some Officers. Officers are people who make decisions that affect the whole business (or a substantial part of it). Generally, Staff are not Officers, nor Line Managers or Supervisors.
However, there is bound to be lots of legal fun when the time comes draw the lines between Directors, CEOs and Senior Managers and how each role is defined within the terms of the Act. The legal constitution of the company will be important here, as will their processes of corporate governance and their Board structure.
Officers will no longer have the power to delegate responsibility. Instead they will inherit a formal positive duty to ensure the whole Governance, Risk and Health & Safety structures are functioning well. This due diligence also includes the need to acquire and maintain a current knowledge of workplace H & S issues. Officers will also need to ensure the PCBU has appropriate resources and process to eliminate or reduce risks.
A culture of safety
A new emphasis in on the word engagement. PCBUs now have a duty to engage workers in the entire Health and Safety culture. Simply informing them is not enough; workers are entitled to contribute to the decision making process, raise issues and express their views.
Workers can be represented by Health and Safety Reps (HSRs) and/or Health and Safety Committees (HSPs), and workers must be provided with reasonable opportunities to engage with these on an ongoing basis. Training for HSPs has become a major new focus, with the PCBU being under a duty to provide that training without cost and at normal pay.
Once an HSR has been trained, they can issue a Provisional Improvement Notice (or PIN) in effect giving notice to the PCBU that an item of equipment, for instance, is in their view unsafe and cannot be used. Although the PCBU has the right to appeal, the PIN may not be removed until after the appeal process has been completed.
PINs can be escalated to the status of Improvement Notices, then onto Prohibition Notices. It is these areas that PCBUs are most likely to come into contact with Worksafe, who will provide a team of inspectors to advise and enforce.
Workers have a right to cease unsafe work, although the PCBU can redirect the worker to other tasks that are appropriate for that worker.
Summing Up
In summary, the new Act seems to be placing a lot of emphasis on the concepts of process, due diligence, verification, transparency and reporting. In fact there seems to be a whole lot of sticks, with the carrots few and far between.
In the 'stick' department, the types of offences which can be committed under the Act have been categorised under three headings:
Reckless Conduct (the most serious breach). Penalties include fines of up $3m and/or 5 years in jail for Officers
Failure to Comply with a Duty, Causing an Exposure to Risk. Fines range up to $1.5m, including individual Officer
Failing to Comply with a Duty (a "technical breach") Fines go up to $500k, again including individual Officers
Remember that Officers can be found guilty of a breach, even if the PCBU has not been convicted.
On the positive (carrot) side, of course lots of this is based on common sense, and the government can be applauded for trying to reduce workplace deaths and harm substantially over the next few years.
They appear to be taking a reasoned approach to the legislation, and Worksafe have indicated that their role will be one of mentors and advisers initially. However, it's pretty safe to say that the big sticks will come out after the first few years. Being one of their first test cases taken to court will not be a pleasant experience!

The Health and Peace of Purging Your Business and Your Life

As with any remodeling or moving you are in a position to make decisions for what to keep and what to purge. I have purged A LOT and ARC and Goodwill are very happy with me.
That said, I have also saved some things. Closets were filled with prom dresses, ski jackets, other kinds of jackets and coats. All belonging to my adult daughters. Back packs, purses you name it.
I would line them up on the couch and take pictures and text them to my girls. Do you want these? No. No. No. No. Oh, maybe save the prom dresses for Halloween.
Well, at least my closets are much more open and clear. (haven't tackled coat closets downstairs yet. I didn't dare ask them about the handmade quilts I made for them before they were born. I will give them to their babies. IF they had said 'no' I think I would have cried. I don't think they would but... I didn't give them that opportunity.
And painting and putting new carpet is your home office is also another opportunity. Shelf Help you purchased years ago (and probably never looked at) is old news. Trends change. What worked 5 years ago or less probably isn't working now. I have about 30 3 rings binders in the closet in my office. All empty! I will never have to buy a 3 ring binders again! And 3 big bookcases. Some duplicates. Some never read at all. (I can tell because I open my books wide so the binder is all wrinkled. My mother in law could read a book and you'd never notice because she barely opened it! I was always amazed)
My next project, which will be huge, is the basement. I have so many boxes of awards, elementary school art, report cards (Yes, I have all their report cards!)
In many, many boxes. And you know what? After reading this article I realize they are not going to want any of them. I will keep what I feel is important. Maybe even frame some 'special' art. I have one that Emily did in 2nd grade right above my computer.
And I was planning on finally putting all those growing up pictures into albums. May have to rethink that one.
